Having AI write meeting minutes. Generating reports. Producing analysis summaries.

None of this is unusual anymore. It's convenient, and the quality is high. The formatting is clean, the logic is sound. The file has a date in its name, it's stored in the right folder, placed on a shared drive.

But there's one thing I need you to check.

Who approved that document?

A File's Mere Existence Made It Look "Official"

Here's what happened on our team.

A member was researching past internal documents to inform a business decision, and came across meeting minutes that had been generated by AI. It had a label indicating it was an official document. Conclusions were stated. A date was included.

The moment they opened the file, they already believed it was an official organizational decision. By the time the team discussion began, all four members were operating on that assumption.

Based on that premise, four people spent roughly four hours building an analysis. They integrated multiple perspectives, crunched the numbers, and assembled it into a formal proposal.

Then they submitted it to our CEO, and everything fell apart.

"That was never an official decision."

A single confirmation wiped out the foundation of four hours of work.

Why Did Everyone Believe It?

Looking back, the reasons were simple.

  • It existed as a file. It was saved in a folder with a date attached.
  • It looked polished. It had conclusions and a logical structure.
  • Multiple people reached the same interpretation. All four arrived at the same reading, and no one questioned it.

With a human-written document, you'd naturally ask, "Who wrote this?" or "Which meeting decided this?" But AI-generated documents come out looking complete from the start. That's precisely why the verification step gets skipped.

Our technical lead, Ryo, reflected on the experience:

"I myself promoted an AI-generated document to the status of 'organizational intent' — I triggered the exact same structure as a hallucination, but on the receiving end."

AI hallucination happens on the generation side. It's when a model outputs something that isn't factually true. But what happened here was a receiver-side hallucination — the content of the AI-generated document wasn't wrong. The problem was that we received it as "an official organizational decision." The humans imposed the meaning of "organizational intent" onto what AI had produced.

We read the file's contents as primary source material. But it never occurred to us to verify how that file was positioned within the organization — whether it was a formal decision or just a memo.

A Document Without a Chain of Approval Is Just Text Data

This experience gave birth to a principle.

An AI-generated document without a chain of approval is not an organizational decision.

A chain of approval means this:

  1. Who requested its creation?
  2. Who reviewed the content?
  3. Who signed off and said "this is good"?

If even one of these is missing, the document is nothing more than text data — no matter how polished it looks.

In traditional organizations, document approval processes functioned implicitly. Because it took time for humans to write, any written document naturally carried "someone's intent."

AI changed that assumption. Documents of high quality are generated instantly, without anyone's intent behind them. That's exactly why the chain of approval needs to be deliberately designed.

Your Organization Has the Same Risk

If your organization is using AI-generated documents directly for decision-making, I'd like you to pause and check.

  • Were those meeting minutes reviewed by the participants?
  • Who approved the conclusions in that analysis report?
  • Was that proposal issued as "someone's decision"?

AI-generated documents will only increase from here. That's why "existing as a file" and "being an organizational decision" are completely different things — and your organization needs to make this a shared understanding now.

A document without a chain of approval is just text data.

We learned that lesson at the cost of 4 people × 4 hours.
